The Mid-Atlantic Disc Club is a leader in promoting disc sports
in the DE, MD, NJ, NY, and PA region. It is a Not-For-Profit Organization made up of
volunteers dedicated to providing a warm welcoming yet professional atmosphere for a
variety of disc related events. Recognizing achievement and offering support to local
leaders while setting and maintaining the highest event standards is just part of our
united mission. By joining the MADC, you show your direct support for these and other
efforts that our many volunteers take on. RENEW YOUR PDGA ANNUAL MEMBERSHIP THROUGH THE MADC BY FILLING OUT AN AFFILIATE CLUB RENEWAL FORM! WHY? HERE ARE A COUPLE OF REASONS: 1) $5 OF YOUR RENEWAL WILL BENEFIT THE MADC INSTEAD OF THE PDGA. 2) RENEWING THROUGH THE CLUB PUTS YOU ON OUR CLUB ROSTER WITH THE PDGA AND YOU CAN TAKE YOUR OFFICIALS TEST FOR FREE! 3) YOUR RENEWALS (BOTH THIS FORM AND THE PDGA AFFILIATE CLUB FORM) MUST BE RECEIVED BY FEBRUARY 20TH, 2006 IN ORDER TO PARTICIPATE IN THE PDGA PROGRAM. AFTER 2/20 YOU WILL NEED TO RENEW SEPERATELY WITH THE MADC AND THE PDGA ON YOUR OWN. AHEM, ONE STAMP, ONE CHECK, TWO RENEWALS. |

PDGA MEMBERSHIP APPLICATION (PLEASE SUBMIT TO MADC) Instructions: Please choose the level of MADC Membership to which you will be subscribing. Check the box next to that level. Then insert the amount of your PDGA Membership into the worksheet. Total those two amounts and write into the bottom line.
